Thank you for participating in the Patient Safety Benchmark Peer Reporting Program.

General Instructions

Data entry precision for this program is critical.  Confirm that all data entered is correct prior to submitting the data. 

Assure that the Laboratory Program ID# is the unique ID# assigned to your facility at the time of registration.

Assure that the collection time frame, numerator, and denominator data submitted comply with the individual Patient Safety Indicator (PSI) definition. 

Data Submission Deadline: submit data by the 20th of the month after the defined data collection time frame.

  • Example:  Data collection time period 01/01/22 – 01/31/22
  • Data Submission Deadline:  02/20/22

Please select the patient safety indicator (PSI) for which you are submitting data. A copy of the defined PSI can be accessed via the link listed in the numerator section below.
Data Time Frame BEGIN Date(Required)
Data Time Frame END Date(Required)
Total Corrected Results Definition: Total number of corrected results reported that required issuance of a corrected report. Include all days in the specific calendar month time frame. (Refer to defined PSI: data collection, analysis, and reporting)
Total Tests Reported Definition: Total number of test results reported. Include all days in the specific calendar month time frame.
Total Delayed Specimen Definition: Total number of rejected specimens that required specimen recollection to complete testing ordered. Include all days in the specific calendar month time frame. (Refer to defined PSI: data collection, analysis, and reporting)
Total Specimens Received Definition: Total number of specimens received that were required to perform testing ordered. Include all days in the specific calendar month time frame.
Rate Calculation: (Numerator/Denominator) x 100 equals Occurrence Rate (in percent) Number reported is percent.
Rate Calculation: (Numerator/Denominator) x 100 equals Occurrence Rate (in percent) Number reported is percent.

A percentage greater than 100% indicates you may have transposed the measures used as the numerator and denominator. Please review your entry before proceeding.

Instructions: Document any data collection or interpretation nonconformity that may have impact on data submitted.
