The Student Forum Leadership Award recognizes a student member of the American Society for Clinical Laboratory Science who has demonstrated outstanding leadership and contributed to the growth and development of the ASCLS Student Forum.
Award: All nominees will receive a plaque and recognitiion at the ASCLS Annual Meeting Awards Ceremony where the winner will be announced and receive a plaque.
Nomination deadline: Feb.15
Nominations are made by a constituent society/region. One student member per society/region is allowed. A previous nomination will not disqualify a person for re-nomination.
- The nominating society (constituent society President or Regional Director) completes the nomination form and forwards this document, along with an application coversheet and application form, to the nominee. It is the nominator's responsibility to make sure the society's nominee understands what must be submitted and follows through to ensure that deadline are observed.
- The nominee completes the application coversheet and application form. He/she submits both documents, along with the nomination form, electronically to the Awards Committee at firstname.lastname@example.org. All three documents must be received by February 15. Nominations lacking any form will not be accepted.
- All forms must be typed and submitted electronically to the Awards Committee. CV's are not acceptable. Electronic "signatures" are acceptable on the signature lines for the nominating constituent society President or Region Director, as well as the nominee.
Eligibility: The nominee must be a student member of ASCLS six months prior to nomination. A nominee remains eligible even if his/her membership status changes from "Student" to "Professional" before the ASCLS Annual Meeting.
The Awards Committee reserves the right to withhold the award for the current year if nominees do not meet requirements.
